I created a survey for my classroom parent's to complete using microsoft forms. The link still brings the parents to the survey but the survey is not on the My Forms page anymore.

If you’ve shared a form with a collaborator, they may also have access to the form’s linked spreadsheet. If you want to remove a collaborator, you have to remove them from the form and the spreadsheet separately. In Forms, you must go in to Edit the form, and the first "fill in blank" is what determines the name of the Form. Microsoft, please note the inconsistency!
